Self-Generation Incentive Program Acknowledgement Form

Complete, sign, and submit this form to initiate your tariff change to Net Billing Tariff (NBT) also known as Solar Billing Plan (SBP) to receive the Self-Generation Incentive Program (SGIP) per the requirements of the program as designated by the Center for Sustainable Energy (CSE).

NOTE: This form must be submitted within 7 calendar days of requesting the tariff change via phone to the Customer Care Center for SGIP participation for processing.

Tariff Change Statement

I acknowledge that I am a named, authorized person on the customer account number entered above. By making the selection above, I am indicating that I want to change my Tariff from Net Energy Metering (NEM 1.0 or NEM 2.0) to Solar Billing Plan (Net Billing Tariff: NBT) for the purposes of participating and receiving an SGIP incentive, regardless of tariff grandfather or legacy status. I am aware that the Solar Billing Plan will be the active tariff associated with the account specified above, and I will not be able to revert back to Net Energy Metering (NEM 1.0 or NEM 2.0) for this account.
